Pyrotechnic Effect Sheet

The I.B.M. Preliminary Stage Contest will be held in the hotel ballroom, and the finals will take place at the theater.  Please note that any pyrotechnic devices and effects used must be listed.  All pyrotechnics must be approved by the local fire marshal prior to use, and they reserve the right to not allow the use of any pyrotechnic devices or effects.  If you do not disclose accurately, you may be denied their use.  The definition of "pyrotechnic" has changed in the past few years to include almost everything.  Examples are: flash paper, flash string, flash-pots, open flames (candles, lighters, matches, etc).  Please list any pyrotechnics used in your act, and indicate whether or not you have licenses for the pyrotechnic devices or effects.
